I'm lucky to have a DMV here in the little desert town. She gave me 3 long forms and stated quite clearly they allow no mistakes or corrections so pay attention. She was stern in a DMV way but had a humorous tone in her voice. I'm still trying to get driver development on the phone for a positive answer to getting a physical at Fontana or going back to Bakersfield.

They cant keep your long form you as a driver are required to keep that form in your truck or yiu will be getting a ticket if you are asked and dont have it with you, they make copies and give you the form to keep its yours and must have it to dr in ve a truck and Swift knows this.

If they ask for it and you dont have a ticket unless you already have it on your license I dont yet waiting to get my new 1 before I go to DMV which will suck since I have to get 1 yearly -
i've never been asked for my long form. and i couldn't even tell you where it's at. they seem to get lost within in a month. -
I have had 3 level 3 inspections and one level 1 in the last 2 months in 4 different states, and all that I have been asked for, show the little card.

Swift had a fit when I left it in my old truck when I switched over I left it DM couldnt g et it he was new and I talk to safety to get it and said u gotta have it rhis was a yr. Ago I have never been asked for it only small one just going by what I was told

Snowy...to answer your question, Swift will pay for your physical if you go where they now tell you to go. Some of the terminals have the on site "clinic", some don't. If you said they sent you a 50 day notice you still have lots of time to get it done. Just do it through one of the terminals through the compliance person, not necessarily the safety person. Just go to the window during daytime hours on a weekday, the person there will get you to the right person.

Finally got through to Driver Development who quickly transferred me to safety. Per safety: (just as Innkeeper said) Show up and they will give me paperwork to get physical at Concentra. Finish physical and bring paperwork back to terminal. They will send me to the commercial drivers DMV office who will process paperwork then return to terminal with paperwork. Physical is now done for another year.
While I have been out on injury I missed a simulator class and logbook class so I have to get those done before I head out plus I think I need a Qualcom refresher since I haven't touched one since early February. Hopefully I can get all this done quickly because this starving on work comp is getting old and once I'm released to full duty it stops but waiting for the classes pays zilch. I need miles to make up for the last 5 months of sitting.
