It's possible #3 and 4 injector could of been fueling a hair more fuel or those holes were a hair on the tight side compared to the rest and were the first to grab a piston. No one will really know for sure thou.
Either way it's unfortunate and don't get me wrong, don't wish that on anybody but you play sometimes you have to pay.
Not sure if trucking is just a hobbie for this guy but I truly feel bad for all his misfortunes when it comes to engines. I'm thinking it might be time to backer down a notch and mile out a engine the old fashion way for a change. Good luck and I hope you get it figured out.

if your taking about adjustment, maybe. but I have ran overhead on many engines and never had an issue. the spray pattern on top of those two pistons were off. #3 was replaced right after I installed the engine by the shop I bought it from. he may have screwed it up. but I don't think so. im starting to think the same thing about cutting it back, but I probably wont, it is to much of a good feeling when you have the POWER. there is one thing that may have caused this, and I will know when I get up and running again. but im still leaning toward these reman injectors. I have had too many issues with them this past year or so.

As soon as it was decided to tear it down you should have couriered them injectors to somewheres with a test bench. You mentioned warranty is covering costs, do you not have a pdi flash, or upgraded turbo?

it is a pre detonation problem the injector was not (shutting off) all the way. the heat from that cylinder migrated to the next cylinder. and no not a pdi tune. a haney tune. the boy that the motor came from is standing behind the failure. it's not a cat warrenty. their warrenties are a joke. you have to remember I haul extreme weights where i'm using 95% of engine capacity 95% of the time. if I was haulin regular freight this probably wouldn't of happened as soon if at all.Buckshot7cz Thanks this.

No , not really talkin adjustment. I was saying piston to liner clearance on those two cylinders might of been just a hair tighter, or those injectors might of been fueling a bit more then the rest , who knows really. All I'm saying is with the kinds of weights you pull and the high duty cycle the pedal spends on the rug, not to mention the engines you go thru. It's quite clear to me it's a little hard on parts doing what you do with that kind of horsepower. I like power as much as the next guy and it's a great feeling blowin the doors off some guy with half the load you got but that same feeling gets extinguished quick when the bank accounts dry from constant repairs.
But hey it's truly your business and no one else's so if you got a money tree in the back yard funding all your fun,all the power to ya!! I know I wish I did. Lol!!! -
the money tree is getting pretty bare! I agree about the piston to liner clearance. but I don't think that is the culprit. I've had a lot of trouble from injectors as a lot of people have on here, stock engines and pumped up engines. no one can bench test them that know what their really doing except one I know of. but I don't have the wait time for a set to get to him and back right now. I have admitted it before and admit it now, I WORK an engine hard! have for years. never had the trouble from an engine until this truck. I'm not arguing with ya. I agree with what your saying on some points. call a streak of bad luck, or the truck is cursed, I don't know. but I am stubborn I will figure it out! i'm using the same ecm and cam and turbo that was on my last engine. what happened to it was the block was bad couldn't keep a head gasket in it. it is going to be fixed with some machine shop work and be sitting for a spare when the time comes. but until then life goes on!

Superhauler what size/brand of turbo do you currently run?

it's a b/w similar to a stage 1 with ecm set at about 650hp. it is a design from haney. works well.

Would a periodic temp reading with a laser infared read have caught it? After a heavy pull, read each manifold exhaust and check if any are higher indicating a problem? Just throwing this out, to see if it might help.
Catch it before it gets worse.
