    Um,,, you might want to KEEP your long form or copy of in your truck at all times, or some smart-arsed DOT can and maybe will give you citation for not having it.

    Not a driver yet. Graduated from school in June, got CDL, and did not apply for jobs as I had to finish up business with my old job. I am going to start applying for jobs this week, hence the concern with losing the med card. And I don't have a copy of my long form. When I got my CDL, the DMV asked me for it and I didn't get it back. Were they supposed to give it back to me?

    I got the physical done at Concerta.

    it's already tied into the system.

    i got pulled over in indiana. faxed my new license and card to the company and forgot to put back in my wallet. so i had neither iwth me. but the cop looked it up. knew that i was legal. and sent me on my way.

    i don't think we have to carry the cards anymore. and probably not our license anymore if we know the number. but since we all need identification some times. i'll keep my license with me.

    and noooooooooooooooooo we don't need the long form. i don't know where some of you get that idea. the long form stays home.

    i don't even know where my long form is. 7 medicals and they've all gotten lost.

    I had my medical card attached to my license by the state back in February. So yes, I don't HAVE to carry my short form medical card anymore.

    You never have needed to have your long form with you.

    You are correct, you never needed to carry the long form. That is for the file in the office.

    However, you still need to carry the short form. Just because you filed a self certification with MN does not mean other states have access to that information at this time.
