    I didn't have to re-do my physical or card when I moved from Wisconsin to Florida in 2000, or when I moved back to WI in 2005. Sounds like you have your bases covered no matter what.

    I do not think you would have any problem even with the NY information.

    It can be handled by saying you lived there. The medical card remains valid based on the drivers information, not his license. Howver, in a year or so, that changes as well. It will be tied to the License.
