I think it's just common sense. We have good drivers and we try to be careful about who we hire. If I can't trust a guy why should I have him around in the first place?
Everybody makes mistakes. Assuming the guy survives his mistake I was taught to look more at the driver's attitude toward what he did than the incident itself. If he owns his mistake, if he learns from it and applies it on the job, he probably won't make that same mistake again...if ever.
Of course, if a guy has a "don't give a crap" attitude about what he did, if he lies, or even fails to own up about what he's done, if he tries to shift the blame...he's out of here quick.
We don't have much turnover, mostly retirees and medical problems. We like it that way.
