Don't give your license up...your tickets should have fell off so no need to bring up the past....you eill need to write a statement on why you were off you may have to sign some kind of waiver due to your previous injury...go on craigslist and find a job, if you are a competent individual you will have no trouble getting back in the saddle so stop thinking of a refresher course...tell the company what you can offer if given the chance and do your thing....talk to whoever gives you a road test make them feel comfortable and let them know if you grind a couple you are rusty and just give you a couple of blocks and it will be like riding a bike....be confident dude you are a professional carry yourself as such
