With engine cold start and feel each port on exhaust manifold. They should all warm up fairly equally. Cold port dead injector.

I still wonder if you are starving for fuel. I am not very familiar with the cummins after the big cams but I believe there is a fuel check valve in an elbow at top left of ecm that has a spring and a check valve inside. It won't cost you anything but time, pull that out and take the elbow apart and clean it really good. See in the higher gears you are needing more fuel to provide the torque and if you don't have the fuel pressure or maybe it's blocked and staying in the head too long, you will lose power and it will knock and cutout and do all kinds of crazy things.

I replaced that check valve already that comes out behind ecm I have bypassed all that today I ran straight from fuel pump to a bucket of fuel and returned to bucket of fuel and went down the road same thing I don't think fuel is the problem
-
I even pulled the rocker boxes and checked the fuel crossovers
