I believe, rust on the side of the frame isn't as serious as rust on the top or bottom of the frame rail that supports the weight. The top of the frame under the 5th wheel is a particularly bad place for rust, and I had the frame rail crack on my Western Star just in front of the 5th wheel once. It's not the end of the world, as I had a place put a smaller piece inside the rail, couple of bolts and welded the crack, and didn't have any more problems, but if your frame is rusty, look it over real good, especially near the 5th wheel.

The sides of the frame rails are what carries and supports the weight, the top and bottom, or flange, is what gives you lateral stability. (side to side)
