Another thing to pay attention to, I was getting alot of mail saying that I needed to pay x amount to keep my UCR active, unless you get this directly from the fmcsa, don't listen to it. They're all scams

OOIDA is great. No issues, and they save you time and money in the long run. I went through them. They send you a packet with all the required forms you need to fill out and keep on file for your inspection.
Yes, you have to fill out an application for yourself. I can tell you, after reviewing my application…I’m way overqualified and am an ideal candidate for my company. I hired myself within seconds of reviewing the application.
Go over your truck with a fine toothed comb. Ensure your low air buzzer works, have your fire extinguisher (I have 3), fire extinguisher inside sign, tool kit, road hazard signs and your safety stuff in order, ensure your low air buzzer is working, ####ty horn, lights, windshield washer fluid sprayer working, signals, hazards, DOT vis tape, and everything else ready to go.
