New Entrant Audit

I've been a company driver for 10 months. I've been my own carrier for 7 months by the audit time. Do I need to do anything with the annual review showing tickets/accidents? Thanks

 

does anyone know what "Signed release forms by each driver for release of testing information" supposed to say? I supposed to sign this for myself. Can someone please post the wording that I can use and sign for myself for my audit? Thanks

 

Ever sign a release form when going to work for someone? They're pretty generic, and basically as your own MC, you'd print one out, allowing your MC to acquire said drug testing results, then you sign and date it, and put it in your driver qualification file.

Pretty sure a google search will turn up a few examples. Here is one from the federal DOT themselves.
