Im not to familiar with that area. Is it more hilly or kinda flat? Im pulling bulk out west here up to 105,000 gross and building a truck with 3.73 gears with tall 22.5's or the same as your LP 24.5's and 3.73's is about the bottom of what the heavy haul guys recommended for the weight Im going to be pulling and Im going with a 565/2050 x15 in my truck. Most guys recommend 3.91's.
If you can run a set of 11r24.5's it should drop your RPM's ruffly 80-100 but remember it will make your engine work harder when pulling the weight you do. But remember the tall rubber will raise you an inch or so on your fifth wheel so tou gotta watch clearances and weight too.
As for your temp issuse I got to thinking of airflow thru your rad's. If you havent done it in a while pull them all out and blow air thru them to clean all the dirt out of the air side. Reason for air over water is that when you get it wet it turns to mud and then clings to the fins easier. If that dosent help then maybe it is time to clean the oil cooler as that engine does run cooler temps then the newer emissions trucks. Also the trucks you've compaired numbers with are similar in design right?? Reason Im asking is a heavy haul truck with huge rad's will run cooler then say an areo truck.

Probably dont wanna run any higher (smaller number) gears then sense you're in the hills as it will make it slower in the hills, use more fuel, and irritate you more. Lol. I'd say you're geared about right just need a little help with heat management.

I had a older ford that recieved a 4 core which was about as big you could get into that thing and still clear the fan on it's waterpump. The result was a car that wont overheat even in the most humid of summers on the midatlantic. Arkansas has the fire several months every year into close to the 130's but Maryland at 95 degrees and 90% humidty and sun that wont quit is something special hot. It poaches you alive.
I think you are pretty good for where you are in PA. I never thought I would consider those "Just hills" compared to what I have seen and run out west but it is what it is. There are some pretty good hills on the east coast too.
Your 5th wheel height is probably determined by whatever tire size they put on at the factory. I don't know what sort of airride you are packing back there but if you need a inch to be taken off, you might be able to take it off at the bag without hurting it too much. It's an idea.
I would lean on a newer rad with more cores. Any time you have bent fins or anything that compromises a existing rad, it's just that much easier to get hot. -
The isx has a T stat for the oil cooler that doesent even stat opening till 233 deg.

I gross 96,500 with a KW900 isx550, 3.70 rears, 18 spead eaton. After washing my radiator my motor runs 30 to 40 degrees cooler. I get around 180 to 210 degrees water temp with a clean radiator.

Oh and the highest recorded temperature ever in arkansas was 120f in 1936. More recently the hottest day was in July of 1980 @ 109.
