I would need to check the paperwork for when the truck was taken out of service. Are they looking for the specific items to be listed as corrected?

I am not sure. I am checking on the paperwork now. Are they looking for a specific statement that the problems with the truck have been repaired?

So what was your immediate correctve action? Ongoing plan? Were/are you qualified for the inspections? Who wrote your plan of action?

If it was placed OOS there has to be a repair fixing it. -
but WHY was it placed oos????

I wrote the plan. It was placed out of service in the fall of last year and has been scraped. loose tie downs and fifth wheel weld are cracked, 5th wheel plate is collapsing

This came up in a compliance review? And from this you got a conditional? Did you jump the OOS at the scales? That's the only way they would go with a conditional for one OOS. Black out the names/numbers and post the conditional rating letter from the FMCSA.

What about showing the DOT the Receipts that this particular truck has been scrapped? I don't know what you would put on the report or a future corrective action going forward? I hope you get it figured out, Are your other trucks not getting the green when they rool acrossed the scale because of this? -
I'm confused. Are you saying that the DOT came to audit you at your office and revoked your motor carrier authority because one of your trucks was placed OOS?
