This is why I don't care to haul O.D. loads. I still do from time to time. All the different rules, time and road restrictions, permits written so vague you almost can't help but screw up a little bit. And that little bit can be big money. And that bridge would not have collapsed under that truck. The weight limit per vehicle is 105,000. What happens when multiple vehicles are on a bridge. Bridges are commonly loaded above the posted limit. Probably that bridge should be posted, "one vehicle at a time" if its that bad.

To proceed without any permit at all is clearly the drivers fault. If you don't understand something on the permit stop and get it clarified before proceeding. I gave up on it because the driver is expected take far more risk than he is paid for.

that heavy of a load should of done route survey to determine if it was a viable route. sounds like some one didn't do his job right!
