I had an EGR cooler replaced due to it leaking. Thats where all this trouble began. Ever since then, It wont passive regen and after a forced regen, the soot level goes to about 80% within 100 miles. So here is what we have accomplished, we had the DPF/DOC filter cleaned and replaced, cleaned the egr valve and also replaced the EGR pressure sensor. Verified correct flow on 7th injector as well. Now after all this, same results. 100 miles and soot level at approx. 80%. No check engine lights, truck runs excellent with good power. My forced regen temps are at 921 degrees, with in spec! So we check the DPF pressure sensor. took tubes off and verified all clear, cleaned them to look like new! Here are what my my pressure sensor results are: .27 at 2100 rpm and .12 at idle, this agrees with 80% soot level, however, we know it doesnt have 80% soot. we hook up a manual back pressure guage and run at high RPM, this agrees with DPF pressure sensor at 1.3psi. Everything is pointing to a backpressure issue, but we know the DPF/DOC is good. What on earth could be causing this increase in backpressure? Could a clogged SCR filter do this? Is there a way to check the SCR filter? Volvo with there eservice recommends new injectors. I dont see how new injectors are causing a back pressure issue. ANY and ALL insight would be greatly appreciated. Everything on a forced regen is within spec. Please help!

Yes SCR could be plugged too. It could have reacted the most to the coolant intrusion from the EGR cooler leak.

In your post you said "DPF/DOC filter cleaned and replaced" I'm a little confused. Were they cleaned or replaced?

So this happened days before the EGR cooler went bad.
    What the...

now after speaking with Nasriza, who is unbelievably helpful, Were gonna try a few things that should narrow the issue down. to either SCR or DPF

Could be the DPF filter collapsed inside, did you flow test it after cleaning? . It happened to me twice after turbo failure oil contamination. I got an aftermarket new from Volvo cuz was cheaper, was smoking bad during regen and that one was faulty also. I took it back flow test it was out of Volvo specs, we tested two more new aftermarket , look in volvo flow rate charts, none passed the test, but they still sell them for about 2500$.

It was professionally cleaned, I believe they included flow rates, I would have to check in am. we scoped the filter after it was installed and all looked good. Thats interesting regarding none passed the flow test. We are going to do a manual backpressure test right after DPF per Nasriza's recomendation to eliminate or confirm SCR as problem. If backpressure is still present, gonna tear into DPF again and examine filters. I just need to narrow it down at this point.

They did a pin test also to check for damage inside, stick a long wire pin , inside those small honeycomb holes, randomly across surface, should pass through the filter .
I found a photo on my phone, I opened the dpf in my garage home many times, drinking a beer and scratching my head :)
As you mentioned , but you already cheched , i had problems with tubes get clogged inside, the two little rubber hosses on top and clamps , sensor cracked and harness bad. The dpf harness is cheap about 200 $ and easy to replace, all these are exposed to heat don't last forever.
