    The way to solve this is to criminally charge the ceo with negligent homicide. Training and enforcement or safety policies would no longer be an after thought.

    Actually those things are in place. Complacency of workers is what causes those accidents. If I remember correctly, there are fans on catwalks, but I haven't had a washout there since few months before that happened

    The way to deal with complacency is accountability.

    It's no different than the handful of grain bin deaths that happen every year. Walking down the grain is illegal, entry of the bin is supposed to follow confinded space protocols, respirators are recommended - but every year someone dies. There is much hand wringing and "if only they'd followed protocol ", a fine is issued and business continues as usual. Toss a couple of site managers and the ceo in jail and attitudes would change in a hurry.

    From what heard the victim leaned down and stuck his head in for a look. And supposedly been working there for over ten years.
    Nitrogen causes you to suffocate.

    So you think the ceo of sni should be held responsible for their employees mistakes?

    That was the issue I was going to bring up. Many of those tank wash people are the shiny pennies in the fountain......
    Heck no one sings songs about tank wash employees, not even Hoyt Axton, Jim Croce or George Jones

    Yes, and to a point yes.

    Last winter I failed a guy for an inability to follow oral instructions. Operations overrode me and the training group. Guy lasted 4 weeks before a customer called to ban him from their property.

    18 months ago I was brought in for a meeting about a former trainee. I was asked what I recalled about him, I said he should have had his cdl revoked because he could not make decisions. I don't know what he did, but it was big and my answer made the legal beagel VERY nervous.

    If Mark Rourke or Tom DeSalvi believed they were risking jail time for the actions of their employees we'd see a lot fewer marginal drivers pass and a larger emphasis on retaining drivers.

    Remember a couple years ago when the driver in Colorado slammed into 17 vehicles and killed a bunch of people? The owner of his company should have been a co-defendant, and I'd argue his cdl examiner as well.

    Stuff happens, and I don't know all the details here, but the article makes me think that there were major short comings in procedures and/or equipment. Or staffing levels, or something else, but something is rotten in Denmark and people need to be held accountable.

    Been farmers around here in years past get in trouble insde vertical silos . Some escaped some did not .

    There are a few places that I've been to that used nitrogen. You can't use air pressure to offload a flammable liquid but they can use nitrogen. They also sometimes will put some nitrogen on top of flammables after they load them. In either case they tag the dome lid with a big warning tag showing that nitrogen has been used. The tank wash personnel definitely take notice of these tags
