My state, and I suspect every state, required me to self-certify which category I am using my CDL. Most of us would certify as operating interstate, non-exempt. In that category the state must have a copy of your DOT Medical Card.
For drivers operating without a requirement for a current DOT Medical Card there is a category or two for those operations. If you weren't driving you could have kept getting DOT Medical Cards and submitting them to the state. Or, you could self-certify to the state you are using your CDL in one of the categories that doesn't require a DOT Medical Card.

NY shows the non cdl as canclled and many mention out of state as 'surrendured' when that state issues their cdl; possibly to keep the system users from thinking 2 cdls existed.
