I laminated mine and wear it around my neck along with my TWIC, keys, CDL, truck stop reward cards(4 of them). When I come strutting through the parking lot there is no mistaken that I am a trucker!

I lost my card once, so I carried the long form with me. DOT accepted it.
The laminate isn't so thick that it can't be folded. I had mine laminated at Staples and had no problems folding it afterwards.
No... it's not until you have the fingerless gloves, mirrored shades, faux leather vest, and chain wallet that there's no mistaking it.HotH2o Thanks this. -
I keep copies in my permit binder of my long form, med card, license. Have never been asked to produce the originals{by law enforcement}

I believe I read that you have to have a copy until at least 2015 on your person. Better safe than sorry. -
I've seen long forms with a medical card on the bottom of the form (bigger than wallet size) where you have to fold it up. It's the bottom 20% of the form, which you cut from the form to carry in your wallet.
