I would never replace a manual transmission, especially an 18-speed, with any automated transmission.

What you are going to get when you fire that thing up after that surgery is something that will cause you to turn your back on everything trucking. -
Ultrashifts are junk, trust me.

You probably could. But do you have the money and the time for breakdowns is the question.
It won't be a cheap and easy task. You'll need wiring and the ecm also. And whatever adapters or engineering might be needed.
You'd be better of just buying a truck already equipped. Why would be beyond us. But who are we to question what you want. We all have our wants and needs.Diesel Dave Thanks this. -

It is understandable with your knee issue.
You would have to see what the cost would be and if the truck you have is worth putting that much money into.
Then you would have to find a shop that's competent enough to not just swap the Transmissions but to be able to understand everything electronically that needs to be done.
For sure there would be nothing inexpensive about it. -
