You must apply in person at a DMV office, and you must:
- Pass a DMV vision screening test (only if you are 50 years old or older on the renewal date of your driver license);
- Present proof of your full legal name;
- Present proof of your legal presence in the U.S., identity and date of birth;
- Provide your Social Security Number on the application;
- If your address has changed, provide proof of your residence address;
- If you have a Hazardous Material Endorsement on your CDL, pass a HAZMAT knowledge test, pay the testing fee and have passed a TSA criminal background check within the past 5 years;
- Certify the type of driving you do or will do and whether you meet federal requirements for operating a commercial motor vehicle;
- Submit a copy of a valid medical certificate if DMV does not already have a copy on file. If indicated on the certificate, DMV must also have a valid waiver or exemption;
- Surrender your current Oregon CDL;
- Pay the renewal fee; and
- Have your photograph taken.
After you meet the requirements, you are issued an interim card to use until you receive your license in the mail.
If you surrendered a previous driver license, permit or identification card, it will be hole-punched and returned to you. The hole-punched card is not valid by itself, but may be used as additional proof of

Now my experience the other day was after taking the practice tests on here 15 or so times, I was confident I would pass. I would suggest a read through also of the hazmat section of your states cdl book. Many questions on my test here in Ohio werent covered in the practice tests on here.Last edited: Jul 20, 2014
