I got mine in Georgia, I paid $57.00. I have heard that it is any where between $50 to $100 dollars, depending on where you go. Just remember that you need to keep your reciept, as the expense will be a tax deduction on your income tax report.

Hey bdanning264, I just took my permit test in Minnesota, and although it wasn't all that hard, I would still recommend you get the book and read through the pertainent sections once before you take the test. To save yourself a trip to the license exam station, you should be able to download a copy of the manual from the internet. Just do a Google search and you'll find it. That's how I did it.
There was a LOT of reading. I read the sections for HAZMAT, Doubles and Triples, and Tanker endorsements too. If I had to do it over again though, I would put off reading about the endorsements for now and just spent my time studying the parts I need for the permit.
For a permit in MN, I needed to pass tests for General Knowledge, Air Brakes, and Combination Vehicles. Chances are it's not much different in Wisconsin.
The first time I went in, I passed the General Knowledge and Air Brakes tests, but ended up failing the Combination Vehicles test. It was no problem though. They just had me come back the next day to retake the Combination Vehicles test. I reread that section of the manual and passed it with no problems.
The biggest bit of advice I can give you is to just try to relax and not put too much pressure on yourself, but to also spend enough time to read through the manual. If you do this you should do just fine.

I actually went and got my DOT physical done today, cost me $65 and went to the DMV to take the test and just see where I was without reading. I failed but was only a few questions off, so I got the book and feel like after I read it tonight I should have no problem passing tomorrow then I'll be all set for class!

Each state is different. Call your DMV or check online for the cost.
As for school, MrG could chime in here and tell you what to bring. Also it should be in your letter/email from Mike Millis.
But just a quick stab at it: cdl permit, DOT card, motor carriers atlas, a calculator, a 6" ruler, clothes for the duration (including jacket and rain jacket/suit), work boots, xtra pair of glasses if you wear them, work gloves, a good flashlight, a notebook, a black ink pen, money for food and whatever else you may need.
Not a complete list I'm sure, but just by what I needed when I went to class.
