dunno, i took mine to the dmv office in Bastrop, @ 20 miles east of austin, gave them my med card - the one i carry in my wallet, signed a self cert form, got my card back and left.
that said, i've since changed companies, and the new company had me sign a state self cert form and they were getting it to the state.
