Leave yourself enough space at the stop light so when you start you can take off in 2nd and grab 3rd or skip to 4th before entering the intersection.

Do not shift in the intersection!

 

That shaking was probably your clutch getting fried.

No I have not, you putting that pressure on the truck also is hard on gear lash.

Anytime you bogg that motor you are hard on it and the tranny.

Thats one reason they allow trucks to run at a high rpm.
