Vibration between 30 - 40 MPH

I'd start with drive train behind the transmission... Check driveline, u-joimts, carrier bearing, rear ends, ECT. If that all looks good maybe check the front end, shocks, suspension, King pins, ECT.

If you're near the truck just grab the driveline and give it all you got and you'll feel a very slight clunk if its worn. At 35 to 40mph it would be pretty significant.

Truck has 860k miles and is an automatic. The shaking/vibration is noticeably worse when I am on the accelerator. It does not seem to matter what gear or rpm though. I have went through that range in every gear the transmission would let me.

I just had the king pins replaced and all the tires rotated and balanced. I know I need to replace my shocks. That is next on my fix it list after this issue that popped up.

An out of round tire will still cause a hop or vibration even if properly balanced. But if they are not new tires thats probably unlikely to be the issue, unless it started when you first got the tires.

When looking for a worn ujoint you'll want to grab the driveline as close to the ujoint as possible to reduce the amount of effort needed to find play in it. Also check the slip yoke for excessive wear.
