    ?
I've only taken the written test to get my permit, and once I passed that they cut up my old license.

You need both the card and the long form. Just go back to the doctor's office with the long form and have him print you a card. You need that card with you at all times.

You won't get your official licence until you've been through the company training and passed a road test that may be at the company's facility( if they are a certified 3rd party tester) or back at the local DMV. not necessarily in your state of residence.

There is no legal requirement to carry a card and there never has been one. You do not need a card at DMV either. On the bottom of the last page IS the official medical examiner certification. If you fail you have an unsigned form. When I transferred my CDL to Virginia I gave them the long form and that was what was scanned. Actually Dr Sager at the Hampton Va Veterans Hospital never gave me a card.

Basically, just tell everyone you will be running interstate commerce- not intrastate commerce. That used to only matter with truck permits, I guess its changed a lot though? But you should make you a copy of the long form physical so the company will have one and you will too. As for the card, you dont have to have it but if you dont, you better have the certificate on the bottom of your long form with you where the Dr signed it and tells how long its valid. I always got them to fill out me a pocket card to carry. D.O.T. will ask for it if they check your paperwork.

Non excepted is basically a normal cdl driver. It means (this is from what I remember, there may be more exceptions) that you are not an emergency responder such as firefighter, emt etc. Also believe farmers are excepted. Has to do with what kind of driving you do. Trucking is all basically non excepted. Just go to the dmv, say you want to change your self certify status to interstate non excepted and you'll be good to go

Good Lord. It's a simple form where you check the interstate or intrastate option. Just go in there with whatever paperwork you have. They will copy it and you will fill out the very basic form and that's it.

Yeah, there has been a requirement to carry a medical card for a long time now. I have even been fined for not being able to provide one. An officer may certainly accept a long form instead of a card, but some proof that you've passed a d.o.t. physical has been required for ages.
