I know! I sent it to my girlfriend, and she immediately picked out what was wrong and she knows nothing about trucks.
I had that happen to me once on my straight truck. Had new springs replaced and re-arched and the spring shop had to remove them to get at everything. As usual they fought them tooth and nail to get them off of the stub tube. They put them back on with a ton of antisieze and tightened them back up within their comfort zone. A few days later the quarter fender on the rh side rotated just like that. No damage. Just had to reset it and tighten up the clamp a lot more.
Whats worse who ever fitted it, or paid for it doesn't look over their work or truck too much. Still it may have been sold to him as a new age directional fender, matches the tyres.
Other than the supposed ‘mud flap’ it appears the air lines are intertwined, which is a no-no. Maybe Chinese tires?
