Had a tire company that shall remain nameless put 4 new tires on back axle for me.
I guess the guy was scared they were going to fall off like this story, so he must have hammered the piss
out of them with an air gun. Got about 40 miles away and drivers side tires passed me. Slid to side of road , collected tires, and scouted around back there for the studs; sure enough, you could see they were stretched.
Spent the rest of the afternoon hammering out and replacing all the studs. Both sides. not fun.
Worst part of the story was one tire had bounded over a dividing guard rail and rolled into an oncoming SUV.
$4,500.00 damage, nobody hurt, and it all went back on the Tire Company. Glad I kept the receipt.
So yeah, been there, done that.
They were tight alright.
Tights tight, and too tights broke.
Wheel fell off.

Every tire shop I have had my truck serviced at for at least the last 5 years uses a 6' long torque wrench to do it properly. But there are any number of totally incompetent service companies and personnel out there.

update - this morning the story was clarified BOTH tires/wheels AND the axle detached.
how could the driver check for this? -
About the only indicators I can think of could be a leaking wheel seal or ABS fault. Maybe a vibration coming from the hub before everything comes apart.

The fact that you see a shower of sparks in the mirror and tires passing you would be a clue.
The serious answer is as Northern Mechanic listed above.
I just know the way my truck feels and handles; anything out of the ordinary requires pulling it over and systems checking everything. Once had a driveshaft starting to shake a little.
Found one of the straps over a cap was allowing movement. Turned around and
went back to barn and changed all.
Rather postpone a job than losing a driveshaft in motion.
