I need a DOT long form.I dont work for a company that supplies them and my doctor says I need to bring my own. Boss says the clinic that he usually sends drivers to has them but I would like to go to my own doctor. Boss dont care but dont know where to get one either. Also does the wallet card come with the long form or do I need to find this too?

Not sure what to do with that. I could print it off but I thought I needed a multi page carbon copy. One the doctor keeps, one for the employer, and one I need to carry.

Print out one copy, and have the doc's office make their copy once it's filled out. You get the original, and make a copy for the boss. Make yourself a copy as well, and keep the original in a safe place!

Or I guess they could buy carbon paper.
Make 2 copie and insert the carbons and do it the hard way.

They could. I have a hard time with carbon paper. I just don't write hard enough for it to transfer through well.

OMG, lol. You know how to use a laptop but never heard of a copier?
Your doctor has to be DOT qualified to give you a DOT physical. I suspect he's not since he doesn't have the forms.Brickman Thanks this. -
Didnt think copies were legal. I thought you needed the multi color carbon copy type that ive always seen. My doctor is qualified. He just dont supply the forms any more.

Copies are completely legal. Just make sure you keep a back up copy for yourself.

Yeah, that's how they normally do it. You can also get the card at the same place. It's best to print it on stock card paper so it lasts.
