Workmans Comp claims are available and listed on a regular 19.95 background check. If its old it should not matter anyway. If they are a legit company to work for they just want to make sure your are not injured now. If you lie or misrepresent any prior accidents or injuries and you get injured again they may not have to cover the new injury because you lied about your history. If they are so petty that they would not hire you because of a healed/signed injury I would not want to work for them anyway.
Tell them truth on what they ask for but no need to volunteer anything they don't ask. -

Agree, when I first starting driving, I had to list all previous employer, for the last 10yrs, even if they were none driving positions. I also had to list any gaps in my work history ie, layed off, sick etc...

Either way we just got through an audit and we require 10 year from experienced and 5 from recent graduates....Been doing this for years and have not had a problem, but that still does not mean that we are right or wrong.
The OP states that the Co. that he applying for requires a 3 year work history. We used to, but recently changed to a 5 year for recent grads.

but note the 10 year requirement is only if the applicant operated a CMV .

Your wording is in conflict with the quote.
It states if you are applying as a cmv operator, not past history. -
and this says ??For which the operator WAS an operator of a CMV .
