Hi Guys, I have bought KYU permit last year. But today I received and mail from Kentucky saying that my permit has been canceled and there is fine of $500 for not reporting my tax. I barely went to Kentucky. What should I do in this case? thank you

Aren't you supposed to still file separately with them even if you didn't have any miles in KY?
86scotty, BigBob410, Snailexpress and 3 others Thank this. -
Says when you sign up, “if you do not come to our site, https://drive.ky.gov/motor-carriers/Pages/KYU.aspx and file every quarter, put 0 if no miles, and you don’t do it, the penalty is $500.BigBob410, Snailexpress, ProfessionalNoticer and 2 others Thank this.

You have to file quarterly, even if you have no miles in Kentucky. Use this guys youtube guide to figure it out if you're having trouble. I think he also did one for NM, OR, and NY.
Speed_Drums and ProfessionalNoticer Thank this. -
Have to file NY, NM, KY every quarter whether you go or not.
Snailexpress, ProfessionalNoticer and Team818 Thank this. -
You "barely" went to Kentucky? So you DID go there, but didn't file a quarterly report? You need to file the report regardless of if you went there or not. If you did go there, you need to pay the mileage fees. If you didn't go there, you don't pay anything, but you still need to file the return every quarter.
Stuff like this is why the failure rate for O/O's is over 90%.86scotty, Speed_Drums, ProfessionalNoticer and 1 other person Thank this. -
Failure rate over the last year might be closer to 10%. When the other shoe drops I wonder how it's going to play out. If enough guys paid off their equipment it might not be as bad as downturns in the past.ProfessionalNoticer Thanks this.

Good point but you have Carriers like OP who can't handle the administrative work and won't last.86scotty Thanks this.

I'd call them up and ask if there is a possibility that the fine could be reversed. Maybe if you had 0 miles for the period you are panelized for, there is a chance...but I doubt it. Still, asking doesn't cost.
If not, I'd pay it or else going to Kentucky could be problematic. Also while you call them, ask them to guide you through setting up you online account and don't forget to report the miles next time. I have had my Kentucky account close to 5 years and I don't think during this time I paid close to half of that for all the miles I traveled through Kentucky ever. So their penalty is a little out of proportion but it is what it is.nikmirbre Thanks this.
