Here in GA you can't walk it in anymore. I watched the guy scan it in. He said it takes a couple of days. I was on hometime so when it was time to head out I checked with the company and they said I was good to go. Also checked the dds website and it shows valid.

I finally got a hold of the doctor and supposedly will try to get it uploaded. “It’s a really new system”. Why there hasn’t been an effort to train & update doctors in this state and others by these clowns at FMCSA is beyond me me. It’s the same doctor I have for regular appointments and he’s good at what he does(and he’s not shy about dq’ing people if he sees a problem, but since the takeover by UR medicine in this state the healthcare situation in this state has gone haywire.

then again Albany is a bit infamous at gross incompetence along with FMCSA.

That’s been a big point of confusion lately. From what I’ve been seeing, the medical examiner is supposed to transmit it now, but it doesn’t always go smoothly like people expect. I’d definitely try to confirm it actually went through so it doesn’t turn into an issue down the road.

Well, a week later the system for nydmv was still showing my old card. Turns out when I sent in the new one to my company they ran the mvr after a few days they found it hadn’t changed so they gave me a helpdesk number to call. When a rep answered I found the new card was rejected because my doctor had his medical examiner clearance yanked recently, so basically he want even supposed to be doing physicals.
Luckily my company already started the ball rolling to get me through their clinic to get a new physical.

word to the wise. Dot has been yanking clearances and cleaning house apparently.
Check to see if your doctor is still on that registry because a lot of them are going out the door.

Welp…. The final chapter to this and it was a bit of a shocker.

Went to the local walk in clinic my company uses for physicals, managed to get a one year card(even though this #### “white coat syndrome is gonna be the death of me), and not only did they send it right in from that office they explained the procedure and were super helpful. Also got the actual card and not a poorly photocopied one like my dr’s office gave me.

Went on about my trip, then when I checked my email about 2am, I got a new email alert from nydmv… saying my medical certificate was updated to show the new card….wow. I figured it would be at least a week or so. Even when I was still sending it into the state it wasn’t that fast.

funny thing it this morning I got another email stating they extended the 60 day waiver for cards issued after April 10.

It because of shenanigans like this, and other absurd interpretations by ill-trained medical types (in most cases are not even a medical doctor) that I gave up trucking and my CDL at age 67 years old.
My present physician, who is on the registry complains routinely "they expect these guys to be as fit as the average astronaut"
When I told him the industrial clinic my company forced all of their drivers to use flunked me, he was flabbergasted.

Here we have an individual who bicycles over 1000 miles per year, weight commensurate with height and no clinical reason that he (me) is not physically capable of safely driving a tractor trailer. But the industry, bought and paid for by the medical/pharmaceutical complex would rather have skinny immigrant drivers who cannot speak English instead of someone with 45 years experience with 4 million miles safe driving.

    don't get me started....

I just dealt with this problem a few weeks ago when I renewed my medical cert, not sure *exactly* who my Driver Leader called, but after about a day of pulling teeth, confirming my cert was in fact IN the system it went from “yeah it should show up at some point (up to a month) to “I see it in the system, it should take a week or two” to “yeah I know we said we couldn’t push it through but we did” the last thing I did on *my* end before the office started handling it was calling the FMCSA’s IT desk (who’s number I got by playing phone tag with a dozen different agencies and DMV’s, and a number I unfortunately did not write down) and sitting on hold, before getting emailed confirmation that it WAS valid, and in the system (which apparently would have made it kosher for me to drive, even if I got pulled into a way stations) but Swift needed it to be fully confirmed to be good. You’ve basically just got to keep calling and nagging a dozen different people to get someone to push a button, insanely frustrating and stressful process that you used to be able to expedite by walking into a DMV with your physical and showing it to them, but apparently the government has decided this is somehow more efficient

This rollout has been confusing, and a lot of clinics are still catching up. Here’s what’s actually going on and what you can do right now.

What the “new rule” actually requires
As of 2024, the Federal Motor Carrier Safety Administration moved to electronic medical certification (NRII system):

    • The DOT medical examiner is supposed to submit your medical certificate electronically
    • It goes into a national database that your state DMV can access
    • In theory, you don’t need to self-submit anymore
    BUT in reality:

    • Not all examiners are set up or trained yet
    • Some offices have no idea about the system (like you’re seeing)
⚠️ What’s happening in your case
    Your doctor’s office is likely:

    • ❌ Not registered or trained in the FMCSA electronic system
    • ❌ Or avoiding it because of confusion/liability
    So they’re defaulting to:

    “We don’t send anything anymore”

Which is half true—but not helpful.

✅ What YOU should do (right now)
    1. Do NOT assume it was transmitted
Until you verify, treat it like it was NOT submitted

    2. ️ Contact the New York State Department of Motor Vehicles
    Ask them directly:

    • “Has my DOT medical certificate been received electronically from FMCSA?”
If they say NO, move to step 3

    3. Submit it yourself (backup plan)
Even with the new system, many states (including NY in practice) still accept manual submission.

    • Upload through your DMV online portal (if available)
    • Or bring/email/fax:
      • Medical Examiner’s Certificate (card)
      • Long form if required
This protects your CDL from being downgraded

    4. Verify your examiner is certified
Check the National Registry of Certified Medical Examiners

    • If they’re listed, they’re supposed to be using the system
    • If not → that’s a red flag
    5. For next time: switch examiners
    Find a clinic that:

    • Clearly says: “We submit to FMCSA electronically”
    • Deals with CDL drivers regularly
This avoids this exact headache next renewal
