question about Dot card

    Stupid question regarding how one carries it. I had to fold mine anout 4 times to get it in my wallet. Wondering how other people carry it.

    DOT card is alittle bigger then a credit card.How small is your wallet that u had to fold it 4 times?.My card is laminated.DOT likes everything neat not all crumpled up.

    Long form? In the bag I always have with me.

    Card? In wallet - like patty says, its credit card sized.

    I've had various sized cards throughout the years. My current one, I had to fold three times. But before I did that, I had it laminated at Staples. I'm told that, now it's in the computer system, I don't have to actually carry it on my person, but I prefer to err on the side of caution.... computer systems fail sometimes.

    Long form would be the 8"X11" document from the physician, correct? I keep seeing "DOT physical card" and wonder if I'm missing something. How does one get it in credit card size?

    The last letter I got, Said to keep your card on you until July or so. Not all states have adapted this yet and they are working on that. But Its not a bad idea to keep on you at all times even when its official.... I agree computer issues plus its the government.

    You should have both the long form and the card. The card is what you need to certify with your state DMV and show the DOT man if he asks to see it. The long form is for your records. You need to keep that with you in case they need to verify the card.

    All I have is a four page form that the Doc and I filled out. DMV made a copy of it and said I was good to go. (Got a permit)

    Also I apologize if this constitutes thread jacking...

    My DOT card is a lot bigger then a credit card. It is Laminated though.
