If one is an OTR driver which option do you mark on your Self-Certification Affidavit? #1 "Non-Excepted Interstate" looks to be the answer. The other options are either INTRASTATE or "No Medical Required". I just want to make sure before I email everything off and make a mistake.

Yes, intra is one state not crossing borders, exempt is farm and such. You need a card so you are non exempt. And you cross state lines so you are interstate

Call your DMV... Google "your state" DMV Self Certification. They should have a phone Number listed. If you happen to mark the wrong one it may cost you later... NC had 4 to choose from. I had to call them to make sure I got the right one.

I found this YouTube video from New England explaining the 4 options. It's clear and probably applies to your state as well. The video agreed with my state.

Not only that, one has to make doubly sure they even receive it, and even that's no guarantee that lazy schmucks at the DMV won't get distracted by a shiny object and forget to process it.
