Sent you a PM, but Sergio's in Simi is a drive (Uses Arleta DMV, at least a month for appointment), there is OC Truck Rentals (Uses Gardena CDT unknown appointment wait times, and Arleta), and there are a few complaints about this place, but you can take the Pearblossum Highway from Victorville and see if Hi-Desert can get you into the Lancaster DMV (Not sureof appointment wait times). The challenge will be getting a DMV Appointment, it is tough tying up a truck and driver for a day to sit at DMV hoping someone will be a no show, or they can fit you in In SoCal getting a behind the wheel appointment usually takes weeks. I have not been to the DNV CDTC Facilities in Fontana or Gardena, but you can always check for appointments online.

Riverside does not offer Commercial Skills Tests. The only offices that offer Commercial Skills Testing in SoCal are Arleta, Bakersfield (Central Valley area, but you can get there easily on the 58) El Centro, Fontana CDTC, Gardena CDTC, Lancaster, and Ventura.

That depends on the physical.Some are more stricter then others.If you can't pass then yes you will get disqualified.This profession requires a lot of using your back.If you hurt your back this time you may never fully recover.

You want to be careful about back pain when you're out on the road. Three years ago I was trying to make a delivery to the Associated Foods Distribution Center in Ogden, Utah.
My back and sciatic nerve were hurting so bad that I had to have an ambulance take me to the hospital. $1300 for the ambulance ride and $1300 for 2 1/2 hours in the emergency room.
Luckily I had relatives nearby that picked me up and took me home. I chose not to file a workman's comp claim because I thought it would prevent me from getting future employment.
I wouldn't do long haul if your back isn't healed up. -

To the OP, I would suggest that you not try to hold any information back with your DOT physical. You might get by with it. OTOH, it could come back and bite the heck out of you sometime down the road. As an example, you get injured on the job so badly, that you can no longer do the work. You file a worker's comp claim. The worker's comp insurance will dissect your life, including certain medical records, (they get around the confidentiality laws, by making you sign a waiver to them, before they will even talk to you about a claim.)
What are they going to find? Oh many things, such as your back injuries, and they even could go as far as finding the post you made here. Not likely, but possible.
Suddenly, your claim is rejected out of hand due to 'preexisting conditions."
Another scenario, your truck is involved in a fatal accident. Your fault or not, your life is likely to be taken apart in a lawsuit from the decadent's heirs. Uh-oh, you've got injuries you have withheld on your DOT physical?
