Yes, she was ABSOLUTELY correct to fail you. Little things like forgetting to cancel a single are a good indication the driver is either not paying attention or is over whelmed by the amount of information coming in. Either way the driver is likely not ready to make safe decisions. They are REACTING to circumstances, not ACTING to create a safe environment for themselves and others.
If you didn't see the light turn to green, assume it's about to change and be ready to stop. Have a decision point identified. On a road test I will verbally indicate where my go/no go point is, that shows the tester that you're trying to be proactive.
In general, driving more than 5 under the speed limit/speed of the road makes a vehicle an impediment and a danger to others (assuming normal weather). That said, there are roads I will drive 10 below on - due to space, or sight lines, or trying to let traffic clear so I can make a maneuver. It's a conscious choice I'm making. Just like it's a conscious choice when I exceed the speed limit. I'm guessing that you didn't realize what speed the speed limit was.
And that is the problem - you're focused on the details and have lost the big picture. This is how trailers end up taking out light poles. You need to be able to shift AND pay attention to everything else. If you're making these mistakes during the CDL test, on roads that you've been on dozens of times - what's going to happen when you're in a new area? So many drivers pass their CDL test, then come to Schneider orientaiton and can't pass the check ride which is basically a CDL test. The difference is the driver learned how to drive the course and not how to drive the truck.
Finally, using the brakes is fine, just don't lug the RPMS below 1,000.

Long as it was yellow who cares!!! Gas on it, don’t lollygag through it ar 25, yellow is proceed with caution,/get outta the way.
But if you’re scared of a vehicle there’s something wrong.

Taking that test is nerve wracking. Everyone was in your shoes once. You could also consider doing it in an automatic if you think you might miss a gear or grind it again. You will still get the job you want in an automatic.

When I tested the insp didn't care about a little grinding.but they did care about not being out of gear for more then the length of truck.not gonna lie,couple times I just put it into any gear just to be in one.took a second to breathe and went into correct one
