Medical Self-Certification Affidavit 1,2,3,4?

    If one is an OTR driver which option do you mark on your Self-Certification Affidavit? #1 "Non-Excepted Interstate" looks to be the answer. The other options are either INTRASTATE or "No Medical Required". I just want to make sure before I email everything off and make a mistake.

    Yes, intra is one state not crossing borders, exempt is farm and such. You need a card so you are non exempt. And you cross state lines so you are interstate

    Call your DMV... Google "your state" DMV Self Certification. They should have a phone Number listed. If you happen to mark the wrong one it may cost you later... NC had 4 to choose from. I had to call them to make sure I got the right one.

    I found this YouTube video from New England explaining the 4 options. It's clear and probably applies to your state as well. The video agreed with my state.


    If you are 48/50-state qualified & are required to get a DOT Medical you pick option #1.

    Not only that, one has to make doubly sure they even receive it, and even that's no guarantee that lazy schmucks at the DMV won't get distracted by a shiny object and forget to process it. :mad:
