Found a little place near the Petro in Ontario CA, everything went smoothly. Now have to goto the DMV tomorrow to get long form sent in.
I took all the forms from my May 22 2014 DOT physical to the DMV and all she did was copy the wallet card like last time.
NEVER go to your regular doctor for your DOT physical! You want to be able to talk to your physician openly and honestly about any health concerns you may have without it potentially affecting your ability to work. ALWAYS go to another doctor for your DOT physical, and do not volunteer information to them. If they don't ask, they don't need to know. If they do ask, be honest but keep your answers short and to the point. Your job depends on it. Your DOT physical is NOT the time or place to discuss the fungus between your toes or the mole on your back. You don't want to give them ANY reason to issue a 6 month card instead of the standard 2 years. Go to your regular family doctor to have those other concerns looked at when it will not affect your ability to work.
Why do you state that with such absolute authority. My PCP, which I have known and gone to for years has done my DOT exam. That day, nothing about medical history is discussed. He does the form and that is it. He does his best to get me a 2 year card every time. But, noone from my Clinic is on the that Registery, but my Chiropractor is. That is insane.
Next time, Just go to the CVS Minute Clinic web site and look up one of their locations that do DOT physicals. They're certified. Cost appox $95.00
The only thing that I didn't like that changed was they no longer give out that little green medical card to put in your wallet. The lady said they don't issue those anymore and it's just a slip of paper with my long form stapled to it. She said that when I get pulled over I need to hand the officer the slip with the long form now instead of that green card. Does not seem right to me that an officer has access to my long form because who knows how they might interpret the information. Did anyone else have this new protocol stated to them since this new registry went into affect?
I got the long form and wallet sized certificate like always, only thing now when you get re-certified you have to go to the DMV in your home state and submit it, along with some other new form, so the state has an actual copy of your health card on file. They didn't ask me for the long form, nor have I ever shown it to a DOT officer. I do carry it in my briefcase though (long form)
Yup. Card in the wallet right behind the CDL, with the long form in the truck. If I should ever lose my wallet, the long form indicates the length of certification. Would I enjoy handing over my long form to a cop? No. Would it beat being placed OOS for lack of proof of being medically qualified? Absolutely.
