Worst road for potholes in my experience is Highway 63 from Fort Mac to Lac La Biche, AB. It's a lot better now since they've started twining and repaving the main 250km stretch.
Worst road for winter would have to be a tie between 63/881 and Highway 16 from Edmonton to BC border. Always see wrecks in the ditch after a snowstorm.
Worst mountain road in winter in my experience is BC Highway 5 (The Highway Thru Hell as it is more popularly known).

Indiana interstates suck...the state roads however are pretty decent.

I-64 through West VA..........man......40 MPH......really?? This on top of endless hills / mountains. Every minute of it sucks

The worst road? Any road with asphalt laid on top of concrete. Within 5 years the asphalt is always in worse shape then the original concrete.
