I’m filling an application to be kept on file.

The application I found online has a category for accidents during the last three years. When operating under somebody’s authority, a woman driver hit the back of my trailer on the highway. It wasn’t my fault. I didn’t get any tickets. And I have a clean record. But I believe the woman needed medical care?
Do I need to put it on the application?
It was a rear end accident. They just gave me the copy of the report.

    Second question:
I was hired as a driver in the past. I didn’t own my own authority. I was one of the subcontractors. So, should my past employment history include that info as a hired driver when I was self-employed.

Its a injury accident. So. Shes lucky she did not get her head chopped by the ICC bumper.

You account for all time going back 10 years. Working or not.

Fill it out as if you were applying for a driving job. Include anything that you would normally include. It's not like you won't approve your own application because of a 3 year old accident.

Working as a subcontractor should be entered as normal employment by the person/company who you were contracted for. Like if you owned your own truck and leased on to a carrier you would be a subctractor. You would say you worked for that carrier and the position could be 'driver, subcontracter' or even just 'driver' as they paid you to drive.

As for accidents, look up your DAC report (it's free) and get your MVR from the DMV website for the state your license is issued in. Don't mention any accidents not listed on one of those things as there is no real record of you being involved. For any accidents that are listed, be detailed in what happened to stress it wasn't your fault if that is the case. If an accident is on your DAC but not your MVR, specify that as well because the insurance companies care about your MVR not your DAC. Even if an accident is considered FMCSA reportable, it might not be on your MVR if you were not at fault.

    Blackhorse77 Thanks this.

With the set ups, what is meant by Diversity Business?
I answered no. But I am not sure what’s it about.

I wouldn't tell em' anything unnecessary. Only what you need to tell them. Keep in mind that the mega companies check everything, and the smaller companies check PSP and your driving record. But......14 years ago when I signed up with one of the megas I only told em' what I felt they would find out.
