Column"I" tax paid gallons..is where you enter the gallons of fuel you bought, in each state.
Put your total gallons in Indiana.
You should have 0) for Illinois and Wisconsin.
The web form should automatically adjust the tax owed to Illinois and Wisconsin. Then give your total tax you owe or credit you get refunded.
I think you will owe less than $15.00

That option isn’t available in my online IFTA form here in TX. Go with what xsetra is saying. I was going to say to try calling Illinois IFTA, but they are probably closed. I tried checking their IFTA manual, but there’s nothing that clearly explains it.

We only had 387 gallons in Indiana. We drove 1996 miles in all three states.
We need to pay around $13.

I think I am finally understanding how it works. Tax-Paid Gallons is only for the states where you purchased diesel. If you did not purchase diesel in a particular state, then insert 0. What’s strange is that the IFTA in TX does this automatically.
