Rock drilling. Common if you get into gravel. Some tires are worse than others. I'm in my second set of Hankooks and they collect rocks like crazy. Yes it will get casings rejected. Never seen a tire blow from it. Or seen a mto/dot question it. If you're really worried about it some sort of liquid rubber to deal it will make you feel better till it falls out in a few hundred miles. Its about as effective as digging rocks out with a pick.

I had that happen once on a brand new Yoko steer. Not as bad as yours though. I ran it anyways for quite a while, without a problem.
