The M16A1 was having some issues with jamming when cycling a round. Having worked on a lot of them, my opinion is that it was caused by slight variations in ammunition. When your dealing with government purchases, where it's massive amounts bought in bulk, it's bound to have some inconsistencies.
When firing, you would know it was going to jam as soon as the pin struck the cartridge. And sure enough, the bolt didn't travel back hard enough to either eject the spent cartridge, or it wouldn't fully catch the next round, getting it stuck in the bottom of the upper reciever, or at some weird angle in the chamber.cke, Eldiablo, singlescrewshaker and 5 others Thank this. -

I watched a documentary some time ago and something in the action literally melted and fused together lol guy said he couldn’t open the bolt even by stomping on it. I agree with the ammo part, crap ammunition always causes problemscke, singlescrewshaker, jamespmack and 2 others Thank this.

Sounds like it wasn't cleaned properly, and would have to be either an A1 or A2, firing full auto for an extended time. By the time they started making the A3, full auto was no longer an option.
There's 4 gas ports at the front of the upper reciever, behind the hand guards. They let gas and heat escape from the chamber as the bolt cycles. Only way I could see that happening, is if the chamber got so hot, that the bolt sort of welded itself to the chamber. Highly unlikely, and it would take millions of rounds being fired with no maintenance before I could see it as being even possible.cke, Eldiablo, singlescrewshaker and 4 others Thank this. -

That's why a forward assist is a requirement on every one of those guns, contrary to the arguments of some idiots I know who feel only military issue rifles need that.
