Usually a "cough" in an ISX, especially a 2250, is the turbo actuator not open enough.
It happens.
If it starts "coughing" get to a shop if you're able.
Cummins X15 "coughing" intermittently.

Idk, i have a water fuel separate filter on there and the thing thats wierd is that it only does it when like the truck has been shutdown for 10 hrs and its cold. Any shop i spoke with they say if its something serious or whatever, it has to be consistent.

Wouldn’t loosing power be one of the main reasons for a turbo actuator to go out? It wont just cough once in the morning if that went out right?

It's in the wrong position for engine speed/demand.
You'll hear it when you're doing a rolling regen and the rpm gets too low, and it will buck from power loss.
Could be several things. Usually sensor.
One cough here or there doesn't mean pneumonia.
I loved my ISX. Kid I sold it too still has it chugging along. -
Run the piss outta it for 24 hrs. Should be fine.
