I can't speak for those specific medications, but I've been told that anything with a sedating effect (i.e benzos) will likely give you trouble with a medical card.
Otherwise I was also told to make sure your doctor notes that they won't affect your ability to drive.

I don't see anything in your post that will automatically prohibit you from driving. Just list all the meds on the physical and make the correct disclosures to the ME on the form. The ME may or may not give you a card, or may give you a 90-day card until you get the prescribing physician to say you are fit to drive. It's a process. If you are on narcotics to "go to bed" it is a major no go.
Gabapentin will be a fun one to get past your training company's controlled substance policy. Obtaining a medical card is one thing, obtaining employment is another.

I talked to three companies on the phone today. All three said they are fine with the DOT Physical and have no policy of their own.

A lot of companies don’t allow medication use… but , thank me later . This stuff save a lot of men careers. My friend I know don’t leave without it and been tested 5 times already

I have taken gabapentin for decades for nerve pain. Also take some other meds
You will need a note from doctor for gabapentin. As mentioned before it is also used for seizures. Also keep the note with you and prescription. Some peoples body chemistry can also have gabapentin show up in drug tests. But as long as it’s prescr to you and on Dot form it is a acceptable medication
