Always listen to @Goodysnap. He knows what the hell he's talking about.
I'm sure he has seen a whole bunch of trucks just like that one that come in and the owner said no no just replace the head gasket it'll be okay and probably within a few months he's seen those trucks come right back.
My question is is there anything else that engine needs? I don't have the answer but does that whole emission system have to be gone over or something done to it at that kind of mileage?
And how about the fuel system? I understand on those engines everything that's involved in that fuel system is crazy expensive.
The last thing I'll say is those engines are expensive. I would imagine that one probably has a vgt turbocharger. That alone is enough to make a grown man cry.
If it were me I'd be looking at what that whole expense would be, and if anything is needed on that engine beyond the rebuild.

Have your mechanic ever re-torqued the head bolts? They stretch over time and it could be the cause of the gasket failure. Replace the head bolts and the head gasket and any other parts that are suspect. Dis the engine use oil or antifreeze prior to this? If not I would simply replace the bolts and the gasket and return to truckin.
