Are you sure the Dr you went to is DOT certified to give physicals. It sounds to me like the Dr you went to isn’t DOT certified and therefore can’t legally give you a medical card. That’s why they need a letter from your Dr which isn’t going to happen with anyone’s Dr.
It sounds like they just wanted the $ for giving you a physical.

The only possible reason for requiring your GP to write a letter certifying you are cleared to drive is:: 1. You have an extraordinary physical condition requiring an exemption, or 2. You are taking a particular medication that requires your doctor to certify it does not impair your driving ability.

If you passed the DOT physical, they would issue the 2-year certificate right away. From my experience, if they tell you they need a note from your primary care physician, it's likely because you told them you take medications which isn't a bad thing but they want to ensure it won't make you an unsafe driver.
EDIT: Huh, if you don't take meds but they still want a note, that's weird. If they told you that you passed the DOT physical, how is that not sufficient? -
Update: It was two horrible medical examiners. One got in trouble with my insurance company.
