ONLY SOME TRUCKING COMPANIES REPORT ANYTHING TO DAC. Whether you were Driver of the Month every month or you drove off a bridge every week, if your employer doesn't report info to DAC, it won't show up on DAC.

What YOU can see on your MVR may be different than what a future employer can see.

This happens more often than people realize, especially during training.

A few things to clear up:

DAC (HireRight) is not automatic. A company has to actually report something for it to show up. Many carriers don’t bother reporting training incidents once a driver is terminated early.

PSP only shows inspections and DOT-reportable crashes. If law enforcement didn’t submit it as a DOT-recordable crash tied to your CDL, it won’t populate; which can also cause the “couldn’t be verified” message if you had very little time in the system.

Training accidents sometimes stay internal to the carrier. If the truck was towed but no DOT crash report was filed, there may be nothing to pull from FMCSA databases.

Ticket vs crash record – The citation itself would live on your state MVR, not DAC or PSP. That’s the one record I’d make sure to check.

Bottom line: it’s possible nothing “fell through the cracks”; it may simply never have been reported to the systems you checked.

If you decide to come back to trucking, I’d recommend pulling your state MVR and being honest about the incident if asked. Most carriers care more about recent history than a training accident from two years ago.
