After viewing the first link again, by pointing out the violations and suspensions and ODOT stating the bridge is safe, it seems like the fault is being directed towards the driver and/or company and away from other possible contributing factors. Due to the weather here the last couple of days, you could speculate that ice on the bridge may have been a factor.

Yeah that looked like a nasty accident Buckeye.......unfortunately it looks like both the driver AND the company are pretty shady........he just got his license back in what 2009 and has had MULTIPLE DUI's ...not to mention the company has been hit with a lot of compliance issues..........what I find odd is how did the tractor come unhooked and the trailer manage to stay on the bridge.....sounds like some kind of fifth wheel or king pin problem...which is gonna be a maintenance issue......I feel sorry for that drivers family, but it seems to me that he was playing russian roulette with his life.

Ya my first though was probably faulty kingpin. However a driver with that record should not be operating a car let alone a semi...........

There was a similar accident in Grand Rapids MI last summer. 3 Trucks wrecked on the same elevated ramp (US131/I-196) over the period of 1 year. Last "driver" had two different licenses (different names) and company knew it.
Tractor tore lose from the trailer, throwing driver to the ground 35 feet below and tractor falling on him.

Oh....and the driver was legally drunk.

According to the article, the tractor didi NOT come uncoupled from the trailer. Rather, the CAB seperated from the frame and went over the side. The rest of the tractor remained coupled to the trailer.
