i have my new entry audit coming up sometime this year lol. i am operating a box truck under 26,001lbs. i am driving my own truck. do i need driver qualification files on myself? that will look real funny me applying to my self! also the forms requesting employment records from previous employers will be from me , about me, they will probably throw them out laughing.
i had a company call me once they learned i was appoved for my MC authority trying to sell their service to get the files dome for me. they said i need them but they want money so i figured to ask here. thx for the help and this was a great post -
To answer your ? Chuck yes you need a driver qualification file on your self and previous employement history. I work closely with the auditor in my area to make sure i am in compliance his name is randy seals i think o your in baltimore so ignore that but feel free to pm with and questions and i can help you get prepared for you audit

Yes, every driver under your authority needs a Driver Qualification File, even if it's just you working for you. Before you can drive under your authority, you have to go through the same screening process every other driver would. So, therefore, you need to 'apply' for the job, get a background check, a pre-employment drug screen, verified past employment (easy, since it was you doing the jobs, if you don't lie about past employers there's no way you can screw up there) for the past three years and driving jobs in the past 10. Also, any failed/passed drug/alcohol tests with previous transportation company. Also, annual review, copy of license, medical form (long and short), driver certification card (basically expires when the first of the two expire), and any qualification paperwork you might have. Also, if your pre-employment drug results must be there BEFORE you drive for yourself. And you must be enrolled in a drug-testing consortium.
I hope you did those last two before you started pulling loads.
I got my letter from PUCO today about scheduling for my New Entrant audit. I'm a little nervous about it, but it's just a one-truck operation and I've followed every rule I can find to a T. Still, it's just the way it is. Hopefully I can schedule it when I planned on being home. -
You can get a driver file kit at the truckstop I made copies so I could reuse it. Foleys is drug testing I use , You will need a vehicle file of maintance, a driver qaulifacation review form and the form the driver signs anually, your inspector qaulifacation form and brake inspector form, a policy for violation of regulations AKa company fine, application, copy of CDL, med card and long form, and a copy of your MSC 90, logbook pages, any accident reports and inspection reports , think thats all they asked at my audit. If you can't find all the forms I mentioned I could probally post or email you a copy. The new entrant form BBB posted looks about the same as the one I had 4 1/2 years ago.
I didn't have a couple of the forms then but it wasn't no big deal, main thing they look for is insurance, drug testing and qaulified driver.
Have all your files out and it will go quickly and you will be done -

The main thing I had to do is enroll into a random drug testing program that I have to pay like $140 a year for. consordium or something like that I can't spell. They will give you 60 days to enroll in one if you haven't done so already, or shut you down.
Have 6 months of log books read to show them!
