Lost my Long form

if and i say if Swift wanted that long form, then you need it. it does have information that the card does not, like the BP readings, the urine test (for sugar), etc,etc.

i am pretty darned sure that when you get to Swift, that day or the day after, you are probably going to be examined again. they may want to compare notes from one form to another.

call your doctor again, i am sure that if he is any type of doctor, he made a photo copy of your long form. someone with half a brain in the doctors office, ought to be able to make yet a copy of that copy for you, and simply note on that, "doctors office copy".

 

It ain't a big deal. Like Gizmo said you'll be taking a new physical. Old ones aren't required. If they did want a copy it can easily be faxed. You ain't going anywhere quick!

One job I quit after 7 months. I had three valid medical cards in my possession. Nobody asks for the old ones.

 

i took my 2nd physical in 2 months first physical was for cdl school and 2nd one was a schneider long form physical,and i also have both medical cards, but i will use the 2 yr one tho.
