Does anyone know how these hooks are mounted behind the bumper? I was thinking of taking a piece of 3/4 steel say 12"x30" or so and using the 8 bumper mounting hole in the front spring hangers. Then put the bumper over it and mount this to the 3/4 plate. Maybe take off the factory tow hook and incorperate a piece from those two bolt to the plate as well. Any ideas? Where can I find these hooks other than the stealership?

it is european tow hook. Really it is not mounted in bumper, becouse bumper is not strong enought (I do not say about military tricks with 8 mm steel bumpers)
really it is mounted on frame mounts with bolts throught bumper. But in Europe it is illegal to put hook on bumper like a weapon of antique greek warships. This hook must be inside bumper, integrated.
On some volvo cabovers it is removable.

My Pete log truck had a tow hook on the front like that. There is a front cross member which is threaded, the bolts go through the tow hook, the bumper and into that plate.

That's kinda how I thought it was. Figured the 3/4 plate for the crossmember.

12" x 30" x 3/4" would make for a serious plate! As Jumbo mentioned, lots of log haulers in our area have that sort of setup. Some have a hefty sized length of cable approx 36" long which is attached to each frame rail, but that will eventually scratch the heck out of a bumper.
That hitch you pictured (or something very similar) was factory supplied on our late 1980s KWs for oilfield use iirc. -
A flat plate mounted behind the bumper is not that strong for the given weight. That plate will weigh about 75 pounds. Angle or channel is a better choice for the cross member.
My truck has a tow hook mounted to each frame rail.
